Prescription Verification Policy

1. Purpose 

Polished Calm LLC (doing business as MShape Beauty) follows all U.S. federal and state laws to ensure safe usage of medical aesthetic equipment. This policy makes sure a valid prescription or medical order is on file before any restricted device or treatment is provided.

2. Scope

• Applies to all staff, contractors, and licensed professionals at Polished Calm LLC and MShape Beauty.
• Covers all FDA-cleared or approved prescription aesthetic devices (e.g., specific lasers, electromagnetic, radiofrequency systems, and microneedling instruments).

3. Core Rules

• Prescription Requirement: No client may undergo a procedure with a prescription-level aesthetic device without a direct order or treatment authorization signed by a U.S. licensed healthcare practitioner (MD, DO, NP, or PA).
• Verification Steps:
• Frontdesk or clinical staff must collect and verify the prescription/order prior to the session.
• Verify the license status of the prescribing practitioner through the state medical board database.
• Documentation: Store the verified prescription securely in the client’s electronic medical record (EMR).
• Audit and Compliance: Review all client files monthly to confirm valid authorization is logged before device operation.
